The Witcher 3: Wild Hunt

On one of the islands northwest of An Skellig, guarded treasure.
Northwest of the Wild Shore signpost in the chest guarded by a cyclops.
On a small island southwest of Free City of Novigrad, in chest guarded by a basilisk.

-!!! Edit: SPOILERS: When in Chapter 2, during the quest "Where is Triss Merigold?" you are brought to the other side of the mist. When there, over to the far right there is a path between two cliffs. Beyond there, you'll eventually reach a small forest with Endregas, after killing them keep moving forward and eventually left there is a pond. Within this pond may be the option to retrieve the sword Beann'shie.
-Note, that it is not possible to craft Beann'shie on chapter 1 due to lack of necrophage eyes (only 1 on these is obtainable, while diagram requires 2)
-From other editor:I amanged to find diagram in ruined elven baths, in the chest, further tests will show if it is the right weapon, or the one mentioned in WARNING section. Testing confirmed, it is the same weapon.
