!!! All overpowered random drop weapons are removed in the pc version and only present in the unpatched xbox360 version !!! !!! This weapon does not exist on PC Enhanced Version !!!
This weapon is the most powerful and one of the best steel swords in The Witcher 2. The diagram to craft Beann'shie can be found in the ruins of the asylum in Chapter I as random loot, but it is extremely rare. you can find a crafted Beann'shie in the same place but it is even more rare.
!!! Edit: SPOILERS: When in Chapter 2, during the quest "Where is Triss Merigold?" you are brought to the other side of the mist. When there, over to the far right there is a path between two cliffs. Beyond there, you'll eventually reach a small forest with Endregas, after killing them keep moving forward and eventually left there is a pond. Within this pond may be the option to retrieve the sword Beann'shie.
Note, that it is not possible to craft Beann'shie on chapter 1 due to lack of necrophage eyes (only 1 on these is obtainable, while diagram requires 2)
WARNING: This has not been tested thoroughly, when playing I chose Iorveth in Chapter 1 and and had the FCR mod installed. It is not clear if these are linked in any way, however I did find the sword in this location, despite claims it does not exist on PC. This is possible to be a bug, although it is not know if it is. The stats also differ from the stats shown, as the damage is only 22 base, and has no freeze but a 10% bleeding and damage reduction on block +5. This may mean it is a bug, or just possibly the sword is nerfed and placed in a different location, not requiring a schematic.
