"Do you really wish to know?" — Spoilers from the books and/or adaptations to follow!
Crones' dagger is a main quest item in The Witcher 3: Wild Hunt.
After progressing far enough in the main story line, Geralt talks to the Crones through Gran, who task him with helping Downwarren's ealdorman before they'll reveal what they know about Ciri. They then order Gran to give Geralt this dagger to show to the ealdorman, so he knows that Geralt was sent by the Crones.
Later, after Geralt completed the task, the ealdorman will then take this dagger and cut off his left ear with it: his payment to the Crones for "helping" the village.
