Family Counselor is an achievement in The Witcher 3: Wild Hunt and requires one to find the Phillip Strenger's wife and daughter.
Theoretically, this one should not be missable as it's part of the main quest line. However, this also means making sure to complete the main quest Family Matters.

- Some have reported bugs in this not popping up. If so, make sure to go and find Tamara and report on her first to the baron, even if you found Anna as well. Once you report Anna's status, the baron will set out to get her without allowing you to also tell him about Tamara.
