Nekker hearts are used as crafting components, alchemy ingredients and also as quest items. Two nekker hearts (with a different icon) in particular are listed as quest items and as such valued at 0 
and a weight of 0. "Ordinary" nekker hearts can also be used to craft the following:
- Fury
- Ysgith rune
- Enhanced White Raffard's Decoction
- White Raffard's Decoction
